Job summary

Priority Dispatch Corp. seeks a Quality Outcomes Specialist to mentor home health staff and enhance OASIS scoring accuracy and documentation. The role involves collaborating with case conferences, huddles, and staff meetings across assigned regions to improve patient outcomes.

You will review charts, support plan of care development, and ensure compliance with HIPAA and regulatory standards while traveling to offices as needed to provide on-site mentorship.

Qualifications

• RN with Florida license and CPR certification required.
• BSN preferred; minimum 3 years’ home care experience.
• OASIS certification required.
• Ability to travel to branches and work weekends.
• Demonstrates strong communication and leadership abilities.

Responsibilities

• Mentor home health staff to improve OASIS scoring and documentation.
• Attend case conferences, huddle calls and staff meetings.
• Monitor and improve outcome performance for regions and clinicians.
• Support mentorship program and travel within region for on-site mentorship.
• Assist clinicians in establishing plan of care and goals for patient needs.
• Participate in chart audits and data collection for QAPI.
