Mercy Home Care is seeking a skilled RN for per diem home health visits in the East St. Louis area. You will provide comprehensive nursing care, manage cases, perform admissions and discharge planning, conduct post-hospitalization assessments, and document all care in the Epic system.

Qualifications

• Licensed registered nurse in good standing.
• Experience delivering skilled home health care is preferred.
• Ability to manage multiple patients with varying acuity.

Responsibilities

• Provide day-to-day home patient care and case management.
• Perform admissions, post-hospitalization assessments, recertifications, and discharges.
• Deliver IV therapy, dressing changes, and general skilled nursing.
• Educate patients and families; document in Epic per policy.
