Duties include admissions to home care services, post-hospitalization assessments, recertifications, discharges, patient teaching, dressing changes, IV therapy administration, and general skilled nursing per policy.

Responsibilities

• Provide day-to-day patient care in the home setting according to policy and procedure
• Case management and total care of multiple patients with different levels of acuity
• Admissions to home care services and post-hospitalization assessments
• Recertifications and discharges
• Patient teaching, dressing changes, IV therapy administration and access site care
• General skilled nursing and documentation in the Epic system
