Qualifications

Responsibilities

• Provide day-to-day patient care in the home setting, including case management and total care for multiple patients with varying acuity.
• Perform admissions, assessments, recertifications and discharges as part of home care services.
• Document appropriately using the Epic system and follow department policies.
