Job summary

Homecare with Heart is seeking a skilled nurse to join our home health team on Fridays from 6a-4p. The role supports patients in their homes under the supervision of an RN, delivering medications, routine care, and education per the plan of care.

Ideal candidates hold an Ohio nursing license, current BLS CPR certification, and at least 1 year of RN/LPN home health experience. We value patient-centered care and collaborative teamwork.

Qualifications

• Licensed nurse in the State of Ohio with good standing.
• Current BLS CPR certification.
• 1+ year RN/LPN home health experience preferred.
• Ability to read and understand plan of care and agency policies.
• Knowledge of medical terminology.

Responsibilities

• Administer medication per standards of practice.
• Document care in progress notes and reports.
• Monitor patient condition and report changes to Clinical Director.
• Support plan of care and collaborate with RN CM.
• Maintain productivity and participate in meetings.
• Adhere to agency policies and HIPAA guidelines.
