Job summary

Homecare With Heart is urgently hiring LPNs to provide in-home skilled nursing across the Ohio Tri-County area. Schedule is Monday–Thursday, 9 a.m. to 5 p.m., with potential additional hours.

A $1,500 sign-on bonus is offered, with a focus on compassionate, experienced care and ensuring quality client outcomes. Responsibilities include medication administration, Plan of Care oversight, patient education, and collaboration with the clinical team.

Qualifications

• Valid State LPN License is required.
• Valid driver's license and use of automobile due to travel involved.
• Annual MUI training as dictated by state requirements.
• Must be able to read and understand Plan of Care and agency policies.

Responsibilities

• Administer medication per standards of practice.
• Encourage self-care and rehabilitative activities.
• Maintain Plan of Care and document care provided.
• Prepare client progress notes and reports.
• Monitor client condition and report changes to supervisor.
• Promote a safe, healthy environment and aseptic technique.
• Collaborate with clinical team to update the Plan of Care.
• Assist client in achieving goals outlined in the Plan of Care.
