Job summary

All Seniors Foundation seeks a Nurse Practitioner (Home Health NP) to deliver advanced geriatric care in patients' homes across Los Angeles County. You will perform in-home assessments, diagnose common conditions, and create individualized treatment plans, coordinating with an interdisciplinary team.

You will oversee medication management, educate patients and families, and ensure quality of life while adhering to California guidelines and best practice standards.

Qualifications

• Valid NP license and CA certification.
• Experience in geriatrics, home health, or primary care preferred.
• Excellent clinical judgment, communication, and patient advocacy.
• Proficient with EHR systems and telehealth platforms.

Responsibilities

• Perform comprehensive in-home health assessments and medical evaluations.
• Prescribe medications, order diagnostic tests, and initiate treatment plans.
• Collaborate with physicians, nurses, therapists, and social workers to coordinate care.
• Educate patients and families on disease management and preventive measures.
• Monitor patient progress, adjust care plans, and maintain thorough documentation.
