Job summary

All Seniors Foundation seeks a Nurse Practitioner (Home Health NP) to deliver advanced geriatric care in patients' homes across Los Angeles County. You will perform in-home assessments, diagnose common conditions, and create individualized treatment plans, coordinating with an interdisciplinary team.

You will oversee medication management, educate patients and families, and ensure quality of life while adhering to California guidelines and best practice standards.

Qualifications

Role Overview

As a Nurse Practitioner (Home Health NP), you will conduct in-home assessments, diagnose common conditions, and develop individualized treatment plans for seniors. You’ll have full scope of practice within California guidelines: conducting comprehensive assessments, ordering and interpreting diagnostics, prescribing medications, and serving as the primary care provider for medically complex elderly patients. Most visits happen in the patient’s home, giving you context most office-based practitioners never see.
