Job summary

All Seniors Foundation in California seeks a Geriatrician to provide expert home-based medical care to aging patients. You will assess complex medical needs, manage chronic conditions, and guide families in difficult care decisions.

You will collaborate with a multidisciplinary team to deliver longitudinal, patient-centered care in homes across the LA area, supporting independence and quality of life in a mission-driven nonprofit setting.

Qualifications

• Board-certified physician with specialization in geriatrics.
• Licensed to practice medicine in California.
• Experience in home-based care models and geriatric syndromes.
• Excellent communication, empathy, and problem-solving abilities.

Responsibilities

• Conduct in-depth medical evaluations and develop personalized care plans.
• Manage chronic conditions, adjust medications, and address acute concerns.
• Collaborate with NPs, PAs, nurses, therapists, and social workers.
• Educate patients and caregivers on disease prevention and health maintenance.
• Document patient encounters and maintain compliance with regulatory standards.
