Job summary

Angels Care Home Health is seeking a Licensed Vocational Nurse (LVN) to administer skilled nursing care under the supervision of a registered nurse for clients in their homes in Conroe, Texas. The LVN will coordinate with the interdisciplinary team, patient/family, and referring agency to ensure quality care.

Requirements include graduation from an accredited LVN program, an active LVN license, a valid driver’s license, and CPR/Hepatitis profile on file.

Qualifications

• Graduate of an accredited school of vocational/practical nursing.
• Current state license as an LVN.
• Current State’s Driver’s License.
• Two years’ experience as an LVN, home health preferred.
• Proof of current CPR and Hepatitis profile.

Responsibilities

• Administer skilled nursing care under the supervision of a registered nurse for clients in their place of residence.
• Coordinate cares with the interdisciplinary team, patient/family, and referring agency.
• Engage in care planning and documentation as required by agency policy.
