Job summary

STAT Home Health in Conroe, TX is hiring PRN Licensed Vocational Nurses (LVN) to provide nursing and supportive care under the direction of the RN, following the Interdisciplinary Plan of Care. This role requires current Texas LVN licensure and at least 2 years of LVN experience; home health experience is preferred.

You will coordinate with physicians and the home health team, document services, and ensure compliance with Medicare/Medicaid regulations.

Qualifications

• Current Texas LVN license.
• Minimum of 2 years as a Licensed Vocational Nurse (LVN).
• Home Health experience preferred.

Responsibilities

• Provides nursing and supportive care under the RN and Plan of Care.

Essential Duties Include

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrates knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN
• Great time management skills

Qualifications

• Current Texas LVN license
• Minimum of 2 years as a Licensed Vocational Nurse (LVN)
• Home Health experience preferred
• Have dependable transportation, driver’s license, and car insurance
