Job summary

STAT Home Health is seeking a PRN Licensed Vocational Nurse (LVN) to provide nursing and support in Conroe, Texas, under the supervision of an RN and an Interdisciplinary Plan of Care.

This role supports patients in their homes, participates in care coordination with physicians and the home health team, and ensures accurate documentation while adhering to Medicare/Medicaid regulations. ESOP benefits are highlighted in this opportunity.

Qualifications

• Texas LVN license required.
• Minimum of 2 years as an LVN.
• Home Health experience preferred.

Essential duties include:

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
• Document all services provided.
• Demonstrates knowledge of Medicare/Medicaid regulations for home health benefits.
• Report changes in patient condition and needs to RN
• Great time management skills

Qualifications

• Current Texas LVN license
• Minimum of 2 years as a Licensed Vocational Nurse (LVN)
• Home Health experience preferred
• Have dependable transportation, driver’s license, and car insurance
