Job summary

Oasis Health Partners seeks a Licensed Practical Nurse (LPN) to provide skilled home health care in the Miami-Dade County area. You will work under supervision of RNs and providers, following the Plan of Care while educating patients and families.

Requirements include graduation from an approved LPN program, Florida LPN license, CPR certification, and at least one year of home health experience. This per diem role offers in-person visits and a flexible schedule Monday through Friday.

Qualifications

• Graduate of an approved practical/vocational nursing program.
• Licensed as an LPN in Florida.
• Minimum of one year of home health nursing experience.
• CPR certification.
• Willingness to align with agency philosophy and goals.

Responsibilities

• Plan and coordinate total client care with RN and provider.
• Follow the nursing care plan for assigned clients.
• Observe and evaluate client conditions and document changes.
• Prepare progress notes weekly with RN supervision.
• Assist with activities of daily living and teach self-care.
• Maintain a safe home environment and assist in complex procedures as needed.
• Coordinate visit scheduling with the nurse and other personnel.
• Report changes promptly to RN and provider.
• Participate in ongoing education and team meetings.
• Perform within scope of training and experience.
• Other duties as assigned.
