Job summary
Matrix Home Health Care is seeking an experienced Licensed Practical Nurse (LPN) for per diem home health visits in Boca/Delray, FL. The role focuses on initiating patient care, completing assessments, and ensuring accurate OASIS documentation for home health patients.
The LPN will provide skilled nursing services, educate patients and families, and collaborate with physicians and the care team to develop and adjust care plans as needed. Per diem schedule with potential for full-time transition.
Qualifications
- Active Florida LPN license in good standing.
- Minimum 1 year of home health experience.
- Strong experience with OASIS documentation.
- Experience with KanTime EMR system preferred.
- Reliable transportation and valid driver’s license.
Responsibilities
- Provide home-based care to patients, often after hospital discharge or facility care.
- Examine patients, perform clinical assessments, document progress, and report changes.
- Complete accurate clinical documentation including OASIS and orders.
- Develop and monitor patient care plans with patients and families.
- Coordinate care with physicians, discharge planners, and interdisciplinary teams.
- Educate patients on plan of care, medications, therapies, and safety.
