Job summary

Matrix Home Health Care in Wellington/Loxahatchee, FL seeks an experienced Licensed Practical Nurse (LPN) for per diem home health visits. You will perform patient assessments, manage care plans, and ensure accurate documentation including OASIS, coordinating with physicians and families.

The role requires Florida LPN licensure, at least 1 year of home health experience, and ability to work independently with reliable transportation.

Qualifications

• Active Florida LPN in good standing required.
• Minimum 1 year of home health experience required.
• Strong experience with OASIS documentation.
• KanTime EMR experience preferred.
• Excellent clinical assessment and critical thinking skills.

Responsibilities

• Provide home-based care to patients in Wellington/Loxahatchee, FL.
• Perform initial and ongoing patient assessments and document progress.
• Complete accurate clinical documentation including OASIS and orders.
• Develop and monitor patient care plans with patients and families.
• Deliver skilled nursing services and case management for assigned patients.
• Educate patients on plan of care, medications, therapies, and safety.
• Coordinate care with physicians, discharge planners, and interdisciplinary teams.
• Communicate with patients, families, and care team; adjust plans as needed.
