Job summary

Granite VNA in Concord, NH is seeking a Licensed Practical Nurse (LPN) for home care, Monday–Friday, 8:00 a.m.–4:00 p.m. Compensation ranges from $24.00 to $33.35 per hour depending on experience.

The LPN will deliver age-appropriate care in patients’ homes under RN supervision and within NH Board of Nursing scope. Qualifications

• Graduate of an accredited school of Nursing with NH license or compact state license through NLC.

Responsibilities

• Follows plan of care developed by the RN in partnership with patient and caregiver.
• Educates patient and caregiver as indicated per care plan.
• Communicates with ordering provider and other health professionals about patient progress.
• Documents nursing interventions and patient responses accurately.
• Participates in interdisciplinary team meetings and agency activities.
• Travel safely within Granite VNA service area.
