Job summary

Granite VNA seeks a Licensed Practical Nurse (LPN) to provide in-home care in Concord, NH and surrounding towns. Schedule is Monday–Friday, 8:00 a.m.–4:00 p.m. Pay ranges from $24.00 to $33.35 per hour depending on experience.

The LPN implements the care plan under the supervision of a Registered Nurse and collaborates with the healthcare team in the patient’s home. The role emphasizes compassionate, age-appropriate care, clear communication with the patient and caregivers, and adherence to

Qualifications

• Graduate of an accredited school of Nursing with NH nursing license or compact state license (NLC).
• One year of recent general medical/surgical or skilled/acute care nursing experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Follows the plan of care (POC) developed by the Registered Nurse in partnership with the patient and caregiver, including documentation of treatments and education.
• Educates the patient and caregiver as indicated in the care plan.
• Communicates with the ordering provider and other health practitioners regarding the POC.
• Communicates patient progress and status with the Interdisciplinary Team during team/patient conferences.
• Participates in interdisciplinary team activities, agency projects, and in-service programs.
• Safely travels throughout the Granite VNA service area.
