Job summary

Granite VNA, Inc. in Laconia, NH is seeking a Licensed Practical Nurse (LPN) to deliver in-home patient care on a Monday–Friday schedule, 8am–4pm, with pay ranging from $24.00 to $33.35 per hour depending on experience.

Reporting to the Home Care Director, you will implement the established plan of care, educate patients and caregivers, document nursing interventions, and communicate with the care team across the service area.

Qualifications

• Valid NH nursing license or compact state license (NLC).
• One year of general medical/surgical or skilled nursing experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Follows plan of care developed by the RN and implemented by the LPN.
• Educates patients and caregivers as indicated by the care plan.
• Communicates with ordering provider and health team members about patient progress.
• Documents nursing interventions, patient responses, and education provided.
• Travel within the Granite VNA service area to provide care.
