Granite VNA seeks a Licensed Practical Nurse (LPN) for home care in Laconia/Meredith, NH. Schedule is Monday through Friday, 8am to 4pm. Pay ranges from $24.00 to $33.35 per hour, depending on experience, with comprehensive benefits and travel time reimbursement.

The LPN delivers age-appropriate home care under RN supervision, documents care provided, and coordinates with the care team to ensure patient safety and independence.

Competenze

• Graduate of accredited nursing program and NH RN/LPN license or compact licensure.
• One year of recent general medical/surgical or skilled/acute care nursing experience.
• Valid driver's license, transportation, and acceptable driving record.

Mansioni

• Follows the plan of care developed by the Registered Nurse in partnership with patient and caregiver, documenting treatments and patient response.
• Educates patient and caregiver about care plan to support independence and safety.
• Communicates with ordering provider and other health professionals about the current plan of care.
• Keeps Interdisciplinary Team informed of patient progress or changes in status within the plan of care.
• Participates in agency projects, QAPI activities, and team meetings as requested by supervisor.
• Safely travels throughout the Granite VNA service area to visit patients.
