Hackensack Meridian Health at Home is seeking a Licensed Practical Nurse (LPN) to provide in-home nursing care across New Jersey with a flexible schedule and meaningful patient interaction. Join a supportive team focused on quality care in patients' homes.

The role requires a minimum of 6 months of nursing experience and strong communication skills, plus proficiency with MS Office or Google Suite. BLS/CPR certification is required within 60 days of hire.

Qualifikationen

• Graduate of an LPN Program or equivalent.
• Six (6) Months Experience.
• Excellent written and verbal communication skills.
• Proficient computer skills that include Microsoft Office and/or Google Suite platforms.

Aufgaben

• Provide home nursing care to assigned patients under the direction of an RN.
• Assess patient acuity and communicate via secure messaging to the physician or case managers.
• Document in the EMR at time of visit and ensure timely admissions per physician orders.
• Prioritize and complete routine assignments accurately under supervision of a registered nurse.
• Teach patients and families based on identified health needs and ensure documentation reflects teaching goals.
