Job summary

Alternate Solutions Home Health of Columbus is seeking a Licensed Practical Nurse to deliver skilled nursing care in patients’ homes across the Columbus area. The role follows a Monday–Friday schedule (8:15 AM–5:00 PM) with on‑call requirements and supports from a full care team.

The position offers a guaranteed base hourly rate plus a generous uncapped bonus structure, mileage reimbursement, and competitive benefits including medical, dental, and vision insurance, paid time off, 401k,

Qualifications

• Licensed Practical Nurse with current state license.
• Minimum 1 year LPN experience; 2 years preferred.
• Home care experience desired.
• Strong communication with patients, families, physicians and co-workers.
• Detail-oriented with nursing judgment.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled nursing care in patients' homes as ordered by the attending physician.
• Administer medications as prescribed by the physician.
• Counsel patient and family on disease processes and how to manage.
• Involve patient and family in plan of care development.
• Help reduce hospital readmissions through education and preventative measures.
• Complete all clinical documentation per agency protocol and Medicare guidelines.
