Job summary

Alternate Solutions Health Network seeks a Licensed Practical Nurse (LPN) to provide skilled home-based care. You will administer medications, monitor patient conditions, and collaborate with physicians, families, and co-workers to develop and follow care plans.

Ideal candidates have 1+ year in acute care, home health experience preferred, and possess a valid LPN license. Travel to patient locations is required; the role supports a comprehensive benefits package and on-call responsibilities.

Qualifications

• Active LPN license in the state of employment.
• Minimum of one year as an LPN in acute care required; two years preferred.
• Home care experience is desired.
• Strong communication with patients, families, physicians and co-workers.

Responsibilities

• Provides skilled nursing care in patient’s home as ordered by the physician.
• Observes and monitors patient conditions and notifies RN/CM of changes.
• Administers medications as prescribed.
• Counsels patient and family on disease and care plans.
• Maintains patient documentation per Medicare/agency guidelines.
• Participates in on-call requirements and team meetings.
