Job summary

Apollo Medical Home Health - a LiveWell Partners Network Agency seeks a compassionate Home Health LPN to work one-on-one with patients in their homes, providing education and skilled nursing to promote independence. The role involves collaborating with a team of clinicians and maintaining a disciplined approach to ensure high-quality care.

Qualifications

• 1 year of LPN experience in home health, med-surg, or ED.
• Current LPN license in the state.
• CPR certification and valid driver’s license.
• Ability to work a flexible schedule.
• Excellent interpersonal skills.

Responsibilities

• Participate in planning and coordination of total patient care with the RN and physician’s plan of care, following the nursing care plan for each patient.
• Accurately report and record the patient’s condition and care, including signs and symptoms indicative of change.
• Prepare clinical/progress notes and summaries.
• Assist the patient with activities of daily living while teaching appropriate self-care techniques.
• Follow universal precautions and all emergency procedures during patient care.
