Job summary

Apollo Medical Home Health - a LiveWell Partners Network Agency is seeking a compassionate Home Health Licensed Practical Nurse (LPN) to work one-on-one with patients in their homes, providing education and skilled nursing to promote independence.

The role is PRN with a minimum of 6 visits per week, in the field, offering pay per visit of $38-$40, mileage reimbursement, and a supportive clinical team to ensure high-quality care.

Qualifications

• 1 year experience as an LPN in a home care, med surg, or ED setting.
• Current state license as an LPN.
• Proof of CPR certification and valid Driver’s license.
• Ability to work and flexible schedule.
• Excellent interpersonal skills.

Responsibilities

• Participate in the planning and coordination of total patient care in conjunction with the RN and the physician's plan of care, and follow the nursing care plan for each patient.
• Accurately report and record the patient's condition and care, including signs and symptoms which may be indicative of change.
• Prepare clinical/progress notes and summaries.
• Assist the patient with activities of daily living while teaching appropriate self-care techniques.
• Follow universal precautions and all emergency procedures whenever giving any aspect of patient care.
