Elk Valley, a part of LHC Group, within UnitedHealth Group family, is seeking a Home Health LPN to provide direct nursing care in patients' homes and to coordinate services within the local service area.

You will work under the supervision of a Registered Nurse or supervisor, documenting visits and supporting care planning, with a focus on high-quality patient outcomes and compassionate care.

Qualificações

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license with vehicle insurance or access to public transportation.
• CPR certification preferred or able to obtain within 90 days of hire.
• Flexible hours to meet client needs.
• Ability to manage multiple tasks.
• Ability to work independently.
• Strong communication, writing, and organizational skills.

Responsabilidades

• Provide high quality clinical nursing services within the scope of practice, as defined by state laws and plan of care, coordinating with the health care team.
• Document patient visits in the electronic medical record per regulatory and payer requirements.
• Participate in care coordination, report needs, attend case conferences, and communicate with patient, family, physician, and team.
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
