Job summary

University Home Care Inc is seeking a skilled Nursing LPN to provide in-home care to patients across Oakland, MI. You will follow physician orders, document care in Kinnser, and monitor patient conditions.

The ideal candidate holds a Michigan LPN license, has home health or skilled nursing experience, communicates effectively, manages time independently, and can travel to patient homes. We offer competitive pay, flexible scheduling, and a supportive team.

Qualifications

Valid LPN license in Michigan.
Experience in skilled nursing or home health care.
Proficiency with Kinnser software or similar home health care documentation systems.
Strong communication and interpersonal skills.

Responsibilities

Provide skilled nursing care to patients in their homes, following physician orders and care plans.
Utilize Kinnser software to document patient care, observations, and clinical data accurately and timely.
Monitor patient health status and report changes to the healthcare team promptly.
Administer medications and treatments as prescribed.
Educate patients and their families on disease management, medication administration, and health maintenance.
Ensure compliance with all regulatory standards and company policies.
