Job summary

Boost Home Healthcare is seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care in patients' homes throughout Michigan. You will administer treatments, perform diagnostic procedures as prescribed, and support patients to maintain independence at home.

The role emphasizes compassionate patient education, accurate documentation, and collaboration with RNs and physicians to ensure high-quality, person-centered care in a home setting.

Qualifications

• Graduate of an approved school of practical nursing.
• Current LPN/LVN license in the posting state.
• Minimum of one year LVN/home health experience.
• Must have current BLS certification.

Responsibilities

• Implement the patient care plan for each home visit.
• Provide skilled nursing services, treatments and preventative care as prescribed.
• Initiate preventive and rehabilitative nursing procedures as appropriate.
• Observe patient signs and symptoms and report changes to the physician and RN.
• Educate patient and family on nursing care needs and home safety.
• Maintain accurate patient records of observations and treatments.
