Responsibilities

• Provide skilled nursing care and treatment in patients' homes per physician orders and care plans.
• Monitor health via vital signs, symptoms, and progress evaluation.
• Administer medications, injections, and treatments as prescribed.
• Coordinate with physicians, therapists and other health professionals.
• Educate patients and families on health maintenance and disease prevention.
• Document patient information accurately per policies.
• Maintain confidentiality and ethical nursing standards.
