Job summary

Elara Caring is seeking a Licensed Practical Nurse to join our growing home health team in Michigan. You will follow individualized care plans, monitor patient progress, educate patients and families, and collaborate with the RN Case Manager to update plans and ensure safe, compassionate care at each home visit.

Required: active LPN license in Michigan, 1 year clinical experience, ability to lift up to 50 lbs, and reliable transportation to travel to patient homes.

Qualifications

• Current State License as LVN/LPN.
• Graduate of an accredited nursing program (Certificate, Diploma, or Associate).
• Minimum 1 year of clinical care experience.
• Must be able to frequently lift, push, pull, and support up to 50 pounds, including patient transfers.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Educate patients and caregivers on self-care techniques to support health.
• Collaborate with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles per agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and ER visits through proactive care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

Education

Licensed Vocational Nurse (LVN) license

Nursing certificate/diploma/associate degree
