Job summary

Elara Caring is seeking an experienced Registered Nurse to support the BAY & MIDFIELD Counties territory in Michigan. You will perform patient assessments, provide nursing care per plan of care, and educate patients and families on care and safety while upholding HIPAA.

The role requires current RN license, 1+ year clinical experience, and the ability to lift up to 50 pounds. A valid driver's license and reliable transportation are required for travel between patient homes.

Qualifications

• RN license must be active in the state.
• Graduated from accredited nursing program (Certificate, Diploma, AS or BSN).
• At least 1 year of nursing experience in a clinical setting.
• Home health experience preferred but not required.

Responsibilities

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.
