Job summary

Elara Caring is seeking a Licensed Vocational / Practical Nurse to join our team delivering right care at the right time in the right place across home health settings.

You will follow individualized care plans, monitor patient progress, and collaborate with the RN Case Manager to update plans as patient needs change, while ensuring safe medication administration and clear communication with physicians and families.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Experience in a hospice or home health environment is preferred

Responsibilities

• Follow each patient's individualized care plan and physician orders
• Monitor patient progress and report changes or safety concerns to the care team
• Help patients and caregivers learn self-care techniques to support their health
• Work with the RN Case Manager to update care plans as patient needs change
• Administer and document medications according to agency guidelines
• Communicate with physicians, patients, families, and the care team to ensure coordinated care
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care
• Deliver compassionate, high-quality care that supports positive patient outcomes
