Job summary

St. Francis Medical Center Home Health is seeking a Licensed Practical Nurse to provide quality in-home patient care within the Post-Acute care network. You will document visits in the EMR and collaborate with the care team to coordinate patient needs.

LA-licensed and experienced LPNs are preferred; the role supports flexible hours and requires reliable transportation for home visits across the service area. Join a caring, patient-centered team within the Optum family.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• LA licensure without restrictions and a minimum of one year experience as an LPN.

Responsibilities

• Provides high quality clinical services within the scope of practice and plan of care.
• Documents patient visits thoroughly in the electronic medical record.
• Participates in care coordination and communicates with patient, family, physician and team.
• Function in diverse home-care settings with sensitivity to all clients.
