Job summary

UnitedHealth Group seeks a Licensed Practical Nurse to deliver quality patient care in home health settings. You will follow the plan of care and infection control procedures to support patients and families in their homes.

Responsibilities include documenting visits in the electronic medical record, coordinating care with the care team, and communicating effectively with patients, families, physicians, and case managers. You will adapt to various home environments and needs.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and reliable vehicle or access to public transportation.

Responsibilities

• Provide high quality clinical services within the scope of practice and plan of care, coordinating with the health care team and adhering to infection control procedures.
• Document patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Actively participate in care coordination, report patient needs and changes in condition, and communicate with patients, families, physicians, other clinicians and case managers.
• Function in diverse home settings and with clients of all ages, races, creeds and backgrounds.
