Responsibilities

• Provides high quality clinical services within the scope of practice, per plan of care and infection control procedures.
• Documents patient visits thoroughly and completely per regulatory and payer requirements in the EMR.
• Participates in care coordination, communicates needs and changes in condition to the team and family.
• Functions in diverse home situations, ensuring respectful and safe care.
