Job summary

UnitedHealth Group is seeking a Licensed Practical Nurse to provide direct nursing care in patients' homes under physician orders and document visits in the electronic medical record. You will coordinate other patient care activities and support the care team within the geographic service area.

Under the supervision of a Registered Nurse, you will ensure infection control, communicate effectively with patients and families, and contribute to a high standard of patient care.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to obtain within 90 days of hire.
• Ability to work flexible hours and manage multiple tasks independently.

Responsibilities

• Provides high quality clinical nursing services within scope of practice.
• Documents patient visits thoroughly in the electronic medical record.
• Coordinates care and communicates with patient, family, physician and care team.
• Adapts to home environments across age groups and conditions.
