Haven Home Health & Hospice in Springfield, MO is seeking a compassionate Licensed Practical Nurse (LPN) to deliver skilled nursing visits in patients' homes, assess needs, document care, and support the care plan with the RN and interdisciplinary team.

Qualifications

• Current Missouri LPN license.
• At least 1 year of LPN experience (home health preferred).
• Home health experience preferred.

Job Summary:

The Licensed Practical Nurse (LPN) provides skilled nursing services to patients in their homes, supporting the established plan of care. You will assess, implement, document, and monitor patient care while working closely with the RN and interdisciplinary team. Your role is essential in ensuring patients receive consistent, compassionate, and clinically sound care in the place they call home.
