Job summary

Haven Home Health & Hospice is hiring a licensed practical nurse (LPN) to provide skilled nursing services in patients' homes in Springfield, MO. You will assess and implement the care plan, document care, and work closely with the RN and interdisciplinary team to deliver compassionate, clinically sound home-based care.

Expect a salary range of $54,600 to $72,800 annually, with PTO, a 401k match, a company vehicle (pending availability), and flexible scheduling to fit your life outside work.

Qualifications

• Current Missouri LPN license.
• At least 1 year of recent LPN experience; home health preferred.

Responsibilities

• Recognize and report changes in patient condition to the RN supervisor.
• Complete timely, accurate clinical documentation.
• Support and carry out the patient’s plan of care alongside the RN.
• Collaborate with therapists and other clinicians to ensure continuity of care.

Job Summary:

The Licensed Practical Nurse (LPN) provides skilled nursing services to patients in their homes, supporting the established plan of care. You will assess, implement, document, and monitor patient care while working closely with the RN and interdisciplinary team. Your role is essential in ensuring patients receive consistent, compassionate, and clinically sound care in the place they call home.

Duties and Responsibilities:
