Responsibilities

• Monitor patient condition and report changes promptly to the RN and physician.
• Educate patients and families on care plans, medications, and safety at home.
• Document visits thoroughly and timely.
• Build relationships with patients week after week.

Schedule

• Primarily Monday through Friday, with the flexibility to structure your day around your patients rather than a shift clock
• Full time: two weekends per month, with a weekday off following each weekend worked
• Part time: one weekend per month

What We're Looking For

• Active LPN license in Oklahoma
• Home health experience is a plus, but not required. We welcome LPNs from hospital, facility, or clinic backgrounds who are ready for a change
• CPR certification
• Comfortable working independently and managing your own schedule
• A genuine desire to provide one-on-one, relationship-based patient care
