Job summary

Interim HealthCare of Oklahoma City is seeking an LPN to provide skilled nursing visits in patients’ homes, working within a full care team. You’ll administer medications and wound care, monitor conditions, and educate families, building meaningful patient relationships in a community-based setting.

Responsibilities

• Provide skilled nursing visits to patients in their homes under the direction of the RN case manager.

• Monitor patient condition and report changes to the RN and physician promptly.
• Educate patients and families on their care plan, medications, and home safety.
• Document visits thoroughly and in a timely manner.
• Build real relationships with the patients you see week after week.

Schedule

• Primarily Monday through Friday, with the flexibility to structure your day around your patients rather than a shift clock
• Full time: two weekends per month, with a weekday off following each weekend worked
• Part time: one weekend per month

What We're Looking For

• Active LPN license in Oklahoma
