Job summary

Interim HealthCare of Oklahoma City is seeking an LPN to provide skilled nursing visits in patients' homes, under RN supervision. You will manage medications, wound care, and care plan education while documenting visits and ensuring timely reporting of changes to the healthcare team.

This role emphasizes autonomy, relationship-based care, and the ability to structure your day around patients. Eligible candidates hold an Oklahoma LPN license and CPR certification; home health experience is a plus.

Responsibilities

• Monitor patient condition and report changes to the RN and physician promptly.
• Educate patients and families on their care plan, medications, and home safety.
• Document visits thoroughly and in a timely manner.
• Build real relationships with the patients you see week after week.

Schedule

• Primarily Monday through Friday, with the flexibility to structure your day around your patients rather than a shift clock
• Full time: two weekends per month, with a weekday off following each weekend worked
• Part time: one weekend per month
