Job summary

Interim HealthCare of Oklahoma City is seeking an LPN to provide skilled nursing visits in patients\' homes, working under an RN case manager. You will administer medications, perform wound care, and monitor patient conditions with prompt reporting.

You will educate patients and families on care plans and home safety, document visits in a timely manner, and build long-lasting relationships with patients week after week. Flexible scheduling supports autonomy and work-life balance.

Qualifications

• Active LPN license in Oklahoma.
• CPR certification required.
• Home health experience is a plus, but not required.

Responsibilities

• Provide skilled nursing visits to patients in their homes under the direction of the RN case manager.
• Administer medications, wound care, and other treatments as ordered.
• Monitor patient condition and report changes to the RN and physician promptly.
• Educate patients and families on their care plan, medications, and home safety.

Schedule

• Primarily Monday through Friday, with the flexibility to structure your day around your patients rather than a shift clock
• Full time: two weekends per month, with a weekday off following each weekend worked
• Part time: one weekend per month

What We're Looking For
