Job summary

Interim HealthCare of Oklahoma City is seeking an LPN to provide skilled nursing visits in patients’ homes. You will work with an RN case manager and care team to deliver high-quality, one-on-one patient care in the OKC metro.

Qualifications

Responsibilities

• Provide skilled nursing visits to patients in their homes under the direction of the RN case manager.
• Administer medications, wound care, and other treatments as ordered.
• Monitor patient condition and report changes to the RN and physician promptly.
• Educate patients and families on their care plan, medications, and home safety.
• Document visits thoroughly and in a timely manner.

Schedule

• Primarily Monday through Friday, with the flexibility to structure your day around your patients rather than a shift clock
• Full time: two weekends per month, with a weekday off following each weekend worked
• Part time: one weekend per month
