Interim HealthCare Home Health and Hospice in Grand Rapids, MI seeks a Home Health LPN. This role offers scheduling flexibility and work-life balance, allowing you to set your own days while delivering patient-centered care.

As a Home Health LPN, you will work with a team to provide in-home care after hospital discharge, manage vitals, document progress, and educate families on care plans. A Michigan LPN license and CPR are required, with at least one year of experience.

Qualifications

• Active, unrestricted LPN license in Michigan.
• At least one year of clinical experience as an LPN.
• Current CPR certification.

Responsabilités

• Provide home-based care to patients with illnesses, injuries and chronic diseases under RN supervision.
• Collaborate with team to deliver quality patient care in the home.
• Take patient vitals, perform assessments and document progress.
• Assist with patient plans of care and teach self-care techniques.
• Educate families on care plans, medications and home safety.
• Coordinate care with interdisciplinary team and plan for discharge.

Home Health NurseResponsibilities:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
• Educate family on the patient’s plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge

A few must-haves for Home Health Nurses:

• Licensed Practical Nurse (LPN) with active, unrestricted license in the State of Michigan
• At least one (1) year of clinical experience as a Licensed Practical Nurse (LPN)
• Current CPR Certification
• Reliable transportation with a valid state driver's license and current car insurance
