Job summary

Interim HealthCare Home Health and Hospice is seeking a Home Health Licensed Practical Nurse (LPN) to provide patient-centered care in the home setting in Michigan. You will operate with flexible scheduling and the autonomy to structure your days while maintaining high-quality clinical standards.

The role requires an active MI LPN license, CPR certification, and strong collaboration with a multidisciplinary team to support patients after hospital discharge and manage ongoing care at home.

Qualifications

• LPN license in Michigan (active and unrestricted).
• CPR certification current and valid.
• At least 1 year of clinical LPN experience is preferred.
• Reliable transportation with a valid driver’s license and auto insurance.

Responsibilities

• Provide home-based care to patients in the home setting under RN supervision.
• Collaborate with physicians, RNs, CNAs and aides to deliver quality care.
• Take patient vitals, perform assessments, document progress and report changes to RN supervisor.
• Assist RN with patient plan of care and teach self-care techniques to patients.
• Educate families on the plan of care, medications and home safety.
• Coordinate patient care and discharge planning with the interdisciplinary team.

Home Health NurseResponsibilities:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care

A few must-haves for Home Health Nurses:

• Licensed Practical Nurse (LPN) with active, unrestricted license in the State of Michigan
• At least one (1) year of clinical experience as a Licensed Practical Nurse (LPN)
