Job summary

Interim HealthCare Inc. is seeking an LPN Visit Nurse for in-home home health care. You will provide direct patient care per the plan of care and nursing scope of practice, while closely monitoring progress and recovery in patients' homes.

The role emphasizes skilled assessments, comprehensive care planning, and education for patients and families. You’ll collaborate with providers as patient needs evolve, and help reduce readmissions through preventative care and continuity of care.

Qualifications

• Graduate of a practical (vocational) nursing program for LPN/LVN and have a current license in the state of practice.
• Current unrestricted license to practice as LPN/LVN in the state for this position.
• Current CPR/AED/BLS/First Aid certification.
• Two years of LPN experience, preferably in home health or similar setting.

Responsibilities

• Provide in-home direct patient care according to the care plan while monitoring progress.
• Complete skilled assessments and deliver patient-centered care in the home.
• Develop and implement a comprehensive care plan addressing patient needs.
• Communicate patient conditions and collaborate with providers as needs evolve.
• Document all clinical activities per agency protocol and Medicare guidelines.
• Educate patients and families involved in care.
• Help decrease readmissions by promoting preventative care and continuity of care.

Position Title: Licensed Practical Nurse (LPN) Visit Nurse - Home Health

Status: Full-time, Part-time or PRN

As a Licensed Practical Nurse (LPN) Visit Nurse you will:

• Provide in-home direct patient care according to the provider's prescribed plan of care (and nursing scope of practice) while closely monitoring progress/recovery
• Complete skilled assessments and provide exceptional patient-centered care in the home
• Ensure an accurate evaluation and implement a comprehensive care plan that addresses patient needs
• Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve
• Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
• Provide education to patients and family members involved in patient's care.
• Work to decrease readmissions by promoting preventative care and ensuring continuity of care

To qualify for a Licensed Practical Nurse (LPN) position with us, you will need:

• Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as an LPN/LVN in the state(s) in which he/she practices and have at least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program
• Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations
• Two (2) years of experience practicing as a Licensed Practical Nurse (LPN in a home health or similar setting
