Job summary

Interim HealthCare in Michigan's Grand Rapids area is seeking a Home Health Licensed Practical Nurse (LPN) to provide in-home patient care with a flexible schedule and autonomy. You will work as part of a team to deliver high-quality care while balancing work and life commitments.

The role involves taking vitals, assessments, and patient education under RN supervision, collaborating with RNs, CNAs, and aides, and supporting discharge planning.

Qualifications

• Licensed Practical Nurse (LPN) with active, unrestricted license in Michigan.
• At least 1 year of clinical LPN experience.
• CPR certification.
• Valid driver license and car insurance.
• Reliable transportation.

Responsibilities

• Provide home-based care to patients under the supervision of an RN.
• Collaborate with physicians, RNs, CNAs and aides to deliver quality care.
• Take vitals, perform assessments, document progress and report changes to the RN.
• Assist the RN with the patient care plan and teach self-care to patients.
• Educate family on the plan of care, medications and home safety.
• Coordinate with the interdisciplinary team to plan for discharge.
