Job summary

AccentCare is seeking a Licensed Physical Therapy Assistant (PTA) for the home health setting in the San Ramon office. This PRN role supports Castro Valley, Hayward, Union City, Newark, Fremont with patient care under a supervising Physical Therapist.

The PTA will provide treatment, educate patients and families, document progress, and collaborate with the interdisciplinary team to ensure quality patient outcomes.

Qualifications

• CPR certification is required and must be maintained.
• Must be a licensed driver able to travel to all locations.
• Ability to implement care plans and communicate with the supervising Physical Therapist and physician.
• Ability to educate patients and families in home safety and equipment safety.

Responsibilities

• Provide patient care under the supervision of a Physical Therapist.
• Document treatment, interventions, and patient progress per policy.
• Educate patients and families and adjust treatment as directed.
• Manage caseload and participate in team conferences and in-services.
• Communicate patient status and changes to the supervising therapist.

Education

Associate degree from an accredited school of Physical Therapy
