Job summary

AccentCare is seeking a Physical Therapist to join the Home Health team in the San Ramon area on a PRN basis. The role includes evaluating and treating patients, educating families, and collaborating with an interdisciplinary team across Pleasanton, Livermore, Danville and surrounding communities.

Qualified candidates hold a CAPTE-approved Master’s in Physical Therapy, have at least nine months of experience, are licensed in the state, CPR certified, and able to travel between office locations.

Qualifications

• Master’s degree in physical therapy approved by CAPTE accredited program.
• Minimum nine months experience as a physical therapist.
• Preferred community/home health experience.
• Currently licensed to practice as a physical therapist in the state of operation.
• Must possess and maintain valid CPR certification.
• Must be a licensed driver and able to travel to all business locations.

Responsibilities

• Provides patient care and documents treatment per physician orders and agency policy.
• Performs patient evaluations and selects appropriate diagnostic material.
• Informs physician of assessment results and proposed interventions.
• Develops and documents treatment plans with measurable goals.
• Educates and counsels patients and families.
• Communicates patient status and plan with the care team.
• Ensures treatment aligns with agency and payer requirements.
• Meets productivity standards and manages time effectively.
