Job summary

Ascension at Home, Together with Compassus in Nashville, TN is seeking a Home Health Licensed Practical Nurse (LPN) to support patient recovery at home and help families achieve health goals. You will educate patients, coordinate with the care team, and provide compassionate, patient-centered care.

Qualifications

• Graduate of an accredited LPN program.
• Active state LPN license and CPR certification.
• Valid driver’s license and ability to drive to patient homes.
• Two years of LPN experience in clinical care; home health preferred.

• Educate patients and families on disease management and self-care.
• Document care provided and coordinate with the care team.
