Job summary

Compassus, in partnership with Ascension at Home, seeks a Home Health Licensed Practical Nurse (LPN) to support patient recovery at home and help families stay connected to what matters most.

You will educate patients on disease management and empower goals, delivering compassionate care in home settings. Requirements: practical nursing license, CPR, valid driver’s license, and at least two years of clinical experience; home health preferred.

Qualifications

• Graduate of an accredited school of Practical Nursing.
• Current state LPN license.
• Current driver’s license.
• Current CPR certification.
• TB screen and hepatitis consent/declination.
• Two years of LPN experience in clinical care; home health preferred.

Responsibilities

• Support patient recovery and enable staying at home with family.
• Educate patients and families to reach healthcare goals through disease management.
